The envelopes are now available at all Arizona MVD offices statewide, as well as from participating law enforcement agencies. You can pick up one of these envelopes at the OVPD Main Station, located at 11000 N. La Canada Drive, during normal business hours. No registration or medical information is required — simply ask for one.
